Extra Duty Application
CERTIFIED SUBSITUTES
All certified substitutes need to be registered with Clackamas County ESD.
When you come in to sign up to be a substitute with the Canby School District, please bring along the yellow registration card from Clackamas ESD, proof of completed mandatory trainings, identification for I 9 form, as well as Social Security Card (It is standard practice of the Canby School District Payroll Department to require all new employees to present their Social Security Card as proof of eligibility for employment.)
